Two surgical options have very satisfactory results to those who are not happy with the volume of their buttocks and wish they were bigger: silicone implants and micro fat grafting, that is, fat collected from other areas of the body and injected into the buttocks. It permanently increases the volume of the buttocks. Fat injections last longer in the buttocks than in any other area of the body, since adipose cells have a tendency to accumulate on the buttocks. It models your body. The buttocks will be enhanced not only because of the increase in volume, but also because they will be more balanced with the nearby areas from which the fat that will be injected into the buttocks will be removed.
What does Augmentation with Cohesive Silicone Gel Implants do?
It permanently increases the volume of the buttocks. If your body does not accumulate fat or you keep in shape with exercise and a healthy lifestyle, the only way of increasing the size of your buttocks will be to resort to silicone implants, since there will not be enough fat in other areas of your body to transfer to the buttocks. It improves the outline of the buttocks. The effect is enhanced when combined with liposuction of nearby areas such as thighs and waist. It balances the bodies of body builders whose buttocks cannot be developed as much as the muscles of the rest of their bodies.

Surgery
About 2 hours, both for buttocks augmentation with silicone implants and with micro fat grafting.
1) Microfat Grafting
By means of a cannula that is thinner than the ones used for conventional liposculpture, the surgeon removes the fat from certain areas of the body, such as the lower back, abdomen, waist and hips. He then purifies the fat collected and injects it on the buttocks drop by drop, very close to blood vessels to be sure that the injected fat will have adequate blood supply.
2) Silicone Implants
The surgeon makes a small incision of 2 or 3 inches (5 cm or 7 cm) between the buttocks, right inside the midline. Through this incision, he makes a pocket inside each buttock and places the cohesive silicone gel implants inside each one of them. There are two possible locations for the implants; the location will be decided according to each patient's needs.
Sub Muscular: If you need to enhance the upper part of the buttocks and the lower part has a good volume (this is what most frequently happens), the implants will be located beneath the buttock muscle (gluteus maximus), approximately where jeans pockets would be.
Sub Facial: If you need to enhance the lower part of the buttocks and the upper part has a good volume, the implants will be located between the buttock muscle and its fascia (fascia or aponeurosis is the tissue that covers the muscle).

Hospitalization
No hospitalization is needed; both procedures are done on an outpatient basis. You will be able to leave 4 or 6 hours after the surgery.

The results
Buttocks augmentation with fat grafting produces permanent and satisfactory results. The most important thing is to feel satisfied with your own body. Bandages - After any of the procedures, in order to heal properly, you will have to wear a special bandage on the treated areas for a few weeks. Uncomfortable but necessary.

Bruises and swelling
The swelling will reach its peak during the first three days, then it will diminish progressively during the following three or four weeks. The incidence of bruises is very low, but they will probably appear and they may take 10 days to heal, they may migrate to lower parts of your body before disappearing. Bear in mind that one side may heal faster than the other, so you may notice a temporary asymmetry.

Scars
Microfat Grafting
The resulting scars will be of no more than 1/8 inches (3 mm.) in the areas from where the fat was removed. They heal in 3 days and they soon become very difficult to see if we do not know where they are.
Silicone Implants
The resulting scar will be of 2 or 3 inches (6 cm or 7 cm), but you should not worry about it since it will be concealed between the buttocks. It is very important to keep the area very clean during the healing process.

Recovery
You will have to sleep on your belly until the stitches are removed. After seven days, you will be able to sit again and to return to your job. You will have to wait approximately 30 days before doing any hard work and physical exercise, depending on how well you recover.

Possible risks and complications
It is very unlikely for buttocks augmentation surgery to present any complications. However, every surgical procedure has some risks.
• Asymmetry - Even though the surgeon marks very carefully the areas he is going to treat, there may be asymmetries.
• Adverse reactions to anesthesia.
• Infections, although they are very unusual.
• If bruises get too large, they may require drainage.
• Seromas or fluid accumulation beneath the skin. In some cases, they may require to be drained with a syringe.
• Temporary loss of pigmentation of the skin in the areas treated.
• Pulmonary thrombosis. If a blood clot travels through the bloodstream and reaches the lungs, it may cause a cardiopulmonary arrest. In order to prevent this from happening you should try to leave the bed as soon as you can. This happens very rarely and it is more likely to happen after microfat grafting than when implants are used.
• The implants may puncture or rupture due to an excessive pressure or violent blow. Even though the cohesive gel of the implants does not spread, if they rupture they will have to be replaced.
• After micro fat grafting, there may be an excessive loss of fluids associated to liposuction that may result in a decompensation.
IMPORTANT: A certain amount of the grafted fat may be reabsorbed by the body if the blood supply is not adequate. This is a natural process that may last some months. It does not present any risks or discomfort, but it results in a loss of the volume obtained.
